DOCUMENTS LIST FOR TAX PREPARATION
Personal information
- Your last year income tax return if you are a new client
- Name, address, Social Security number and Date of Birth for yourself, spouse, and dependents
- Banking information for Direct Deposit: Routing #, Account # or bring a canceled check
- Proof of address for dependent children
- Proof of Health Insurance form -1095-A/B/C
- Your State I.D. Card, or Driver’s I.D., Licenses or passport
To report the correct income information – you need to following:
- All W2’s from all jobs held in the tax year, and 1099 Unemployment income
- All 1099’s- INT/DIV/G/S/MISC/MSA/R/C/SSA – Interest /Dividend/Stocks/Bonds etc Business Income if self employed (Accounting Records)
- Gambling/Lottery Winnings and Losses/Prizes/Bonus
- Alimony Income
- Rental Income & Rental Property Expenses
- Foreign Income

- IRA contributions Form 5498 from bank

- Education credit – Needs 1098-T form
